骨科损害控制在骨盆型严重多发性损伤中的应用(附61例报告)

Damage Control Orthopedics for Severe Pelvic Multiple Injury with Hemorrhagic Shock

目的总结和分析骨盆型严重多发性损伤的病情特点,为临床上骨盆型严重多发性损伤应用骨科损害控制提供初步依据。方法对61例骨盆型严重多发性损伤进行回顾性研究,通过ISS评分、损伤部位数、手术部位数、早期复苏输液量、复苏时间、手术时机、住院时间和Matta评分等指标进行量化分析及疗效评估。结果本组61例,ISS评分17～50分,平均27.38分。Ⅰ组、Ⅱ组、Ⅲ组损伤部位数、手术部位数、确定性手术时间、住院时间、Matta评分优良率均无显著性差异(P>0.05)。结论①阐明骨盆型严重多发性损伤的概念;②骨科损害控制有利于骨盆型严重多发性损伤的救治。

Objective To collect the cases of severe pelvic multiple injury,analyze and summarize the feature of its pathogenetic condition,then provide initial evidence for applying damage control orthopedics in treatment of severe pelvic multiple injury.Methods Retrospective study was adopted from 61 patients with severe pelvic multiple injury in our division.The patients were analyzed and appraised by quantizing the index of ISS,injury sites,operation sites,earlier period transfusion quantity and time of revitalization,operation opportunity,length of stay and Matta score.Results There were 61 cases of severe pelvic multiple injury.The ISS of all the cases were between 17 and 50,and the average was 27.38.Among the groupⅠ,Ⅱ and Ⅲ,there was no significant difference in the injury sites,the operation sites,the operation time,the length of stay,and the Matta fineness rate（P 〉0.05）.Conclusion ①The conception of severe pelvic multiple injury is explained.②Damage control orthopedics is helpful in treatment of severe pelvic multiple injury.
