关于产妇生产两难问题及伦理原则的探讨

在产妇的生产方式中,一方面不符合手术指征却滥用剖腹产手术的现状令人堪忧;另一方面在低收入和文化水平较低的家庭中由于经济和传统观念等因素影响,产妇难产却因拒绝剖腹产术而导致死亡的现象也在一定程度存在。总结了产妇生产中医生面临的三种伦理难题,指出临床医生既不能无视剖腹产手术的风险和危害滥施剖腹产手术,也不能盲目迁就失去理性的病人或家属,错失剖腹产抢救手术的最佳时机,置病人或胎儿的健康利益于不顾。而应在有利无伤、尊重患方理性的医学伦理学原则指导下,理性地分析产妇情况,为其提供科学合理的生产方式,以求最大限度地保障母婴的生命健康利益和生活幸福。

This article summarizes three ethical problems in selecting delivery modes. The abuse of caesarean section and difficult labour resulting to maternal death are also analyzed from the perspective of medical ethics, concluding that the puerpera＇s condition should be rationally analyzed and treated under the guidance of the ethical principles of benefit and respecting patients＇dignity, with scientific delivery mode provided and advised, in order to maximize the protection of maternal and child health and welfare.
