颈动脉海绵窦瘘的磁共振诊断价值

Diagnosis Value of Magnetic Resonance Imaging for Carotid-cavernous Sinus Fistula

目的:评价MR、MRA对颈动脉海绵窦瘘的诊断价值。方法:回顾性分析16例颈动脉海绵窦患者的MR、MRA表现,并对比分析6例颈动脉海绵窦的全脑血管造影检查表现。结果:颈内动脉海绵窦瘘MR表现为眼球突出,海绵窦扩大,属支静脉扩张、吻合支扩张及脑挫裂伤。MRA与DSA血管影像表现相一致,但MRA结合常规序列可以观察到血管外结构,DSA电影可动态观察病变血管显影情况。结论:若仅为诊断颈动静脉海绵窦瘘,MRA结合常规MR,其优势大于DSA;但DSA存在介入治疗优势,是目前MR所不能及的。

Objective To evaluate magnetic resonance imaging（MR） and magnetic resonance angiography（MRA） in the diagnosis of carotid-cavernous sinus fistula.Methods 16 cases of carotid-cavernous sinus fistula were retrospectively analyzed in their MR and MRA appearance,in which 6 were examined by digital subtraction angiography（DSA） of head.Results The MR appearance included exophthalmus,enlarged cavernous sinus,engorged branching vein and ramus anastomoticus,and the contusion and laceration of brain,which were consistent with the appearance of DSA.The difference between the two examinations was that the former showed the structure around bloods,while the latter displayed the blood diseases dynamically.Conclusion The combination of MR and MRA is better in the diagnosis of carotid-cavernous sinus fistula than DSA,but inferior to the latter in interventional therapy.
