摘要
目的探讨2型糖尿病(T2DM)患者血清脂联素(APN)与炎性因子的相关性,并观察抗炎药物对糖尿病患者的疗效。方法将2型糖尿病患者分为T2DM合并血管病变组(A组)53例,T2DM无血管病变组(B组)47例,以健康人30例为对照组。分别检测抗炎治疗前后各组胰岛素、糖化血红蛋白(HbAlc)、血脂(1ipids)、游离脂肪酸(FFA)、C-反应蛋白(CRP)、APN、瘦素(Leptin)及肿瘤坏死因子(TNF-α)、胰岛素敏感性指数(ISI)及胰岛素抵抗指数(IR)等。结果ISI、IR、FFA、CRP、APN、Leptin、TNF-α含量均高于对照组(均P〈0.05);抗炎治疗后明显下降(均P〈0.05);APN平与HbAlc、HOMA-IR、CRP及TNF—α呈显著负相关(均P〈0.05)。结论APN是T2DM和动脉粥样硬化的保护性因子,抗炎治疗可能改善T2DM的预后。
Objective To investigate the correlation between serum adiponectin level and inflammatory factors in patients with type 2 diabetes melhtus(T2DM). Methods In our hospital out-patient and patients with T2DM were divided into combined macrovascular T2DM group( A group) 53 cases;T2DM without macroangi-opathy group( B group) 47 cases; 30 cases of healthy people as control group. The contents of insulin, glycosylated hemoglobin (HbAlc) ,blood lipids ,free fatty acids(FFA) ,C-reactive protein(CRP) ,adiponectin(APN) ,leptin and tumor necrosis factor(TNF-α) ,insulin sensitivity index(ISI) and insulin resistance index(IR) were detected. Results ISI,IR, FFA, CRP, APN, Leptin, TNF-α levels were higher( P 〈 0. 05 );anti-inflammatory treatment was significantly decreased(all P 〈 0.05 );APN level and HbAle, HOMA-IR, CRP,TNF-α was significantly negatively correlated( P 〈 0. 05). Conclusion APN is a T2DM and atherosclerosis protection factor, anti-inflammatory treatment may improve the prognosis of T2DM.
