妊娠高血压疾病患者凝血相关指标的改变及临床意义

Change of coagulation in Patients with Hypertensive disorder complicating pregnancy

目的通过对妊娠高血压疾病组(包括妊娠高血压组、轻度子痫前期组、重度子痫前期组)与正常晚孕组(足月)孕妇的凝血功能、血小板及血脂相关指标的检测及统计学分析比较,探讨凝血相关指标的改变与易栓症发生的相关性。方法选择62例正常晚孕组和150例妊娠期高血压疾病患者对凝血酶原时间(PT)、部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FBG)、抗凝血酶Ⅲ(AT-Ⅲ)、凝血酶原时间国际化标准比值(PT-INR)以及血小板计数(PLT)、血小板压积(PCT)、血小板平均体积(MPV)、血小板平均分布宽度(PDW)、血脂指标甘油三酯(TG)、胆固醇(TC)进行检测和统计分析。结果妊娠高血压疾病组与正常晚孕组相比,PT、APTT、PT-INR、AT-Ⅲ、PLT值差异有统计学意义(P<0.05);FBG、MPV、PDW、TG值差异有统计学意义(P<0.05);TT、PCT、TC值各组间差异无统计学意义。结论联合检测凝血、血小板、血脂指标说明妊娠高血压疾病孕妇比正常孕妇具有更高的凝血状态,有血栓形成倾向,并且随着病情加重而加重。

Objective To investigate the connection between the change of coagulation function and thrombosis in Patients with Hypertensive disorder complicating pregnancy.Methods The PT,APTT,TT,FBG,AT-ⅢA,PT-INR,PLT,PCT,MPV,PDW and TG,TC were analyzed in 62 normal pregnant women in the third trimester and 150 pregnant women with hypertensive disorders.Results Compared with the normal pregnant women in third trimester,the level of PT,APTT,PT-INR,PLT,AT-ⅢA in the pregnant women with hypertensive disorders were obviously decreased（P〈0.05）,while that of FBG,PDW,MPV,TG were significantly increased（P〈0.05）.Conclusion There are thrombophilia in pregnant women with hypertensive disorders.
