Onyx胶栓塞治疗脊髓动静脉畸形

Endovascular embolization with Onyx in the treatment of spinal arteriovenous malformations

目的分析Onyx胶栓塞治疗脊髓动静脉畸形的安全性及疗效。方法回顾性分析14例接受Onyx胶栓塞治疗的脊髓动静脉畸形患者的资料,综合血管造影结果及患者脊髓功能改善情况进行疗效评价。结果本组14例患者中,12例经1次栓塞治疗,2例经2次栓塞治疗。栓塞后血管造影显示,7例病变血管完全闭塞(栓塞范围100%),4例大部分闭塞(栓塞范围80%～99%),3例部分闭塞(栓塞范围30%～79%)。平均随访15个月,痊愈2例,好转10例,1例无变化,1例恶化,多数患者(12/14,85.71%)脊髓功能明显改善。结论 Onyx胶是治疗脊髓动静脉畸形安全、有效的栓塞材料。

Objective To observe the safety and efficacy of endovascular treatment of spinal arteriovenous malformations（AVMs）with Onyx.Methods Clinical and radiological data of 14 patients with spinal AVMs treated exclusively with Onyx embolization were retrospectively analyzed.A comprehensive assessment of the efficacy was performed according to angiography results and neurological and functional evaluation.Results Twelve patients underwent once endovascular procedure and 2 patients underwent twice endovascular procedures.Angiography after embolization showed total obliteration in 7 patients,subtotal obliteration in 4 patients and partial obliteration in 3 patients.Clinical follow-up was performed and the mean follow-up duration was 15 months.Improvement in neurological and functional status was found in 12 of 14（85.71%）patients,including 2 of recovery and 10 of relief.No obvious amelioration or exacerbation of neurological and functional status was observed in 1 patient,respectively.Conclusion Embolization with Onyx is a safe and effective method for the treatment of spinal AVMs.
