精神分裂症漏诊脑梗死的原因分析与对策

Analysis and countermeasures of cerebral infarction missed diagnosis in schizophrenia

目的:分析精神分裂症患者漏诊脑梗死的原因,并探讨减少漏诊的对策。方法:对2008年3月至2010年3月来我院就诊的精神分裂症患者127例进行回顾性研究,采用CT和(或)MRI、DWI检查以确诊脑梗死。结果:经CT或MRI、DWI检查确诊脑梗死11例(8.66%),患者均有明显的精神症状,病情与梗死区域的功能有关,进行相应治疗后病情均改善。结论:精神分裂症合并脑梗死临床较少见,尤其是有明显的精神症状,但体征不明显时容易漏诊,临床医师应予以重视,以免耽误治疗时机。

Objective：Analysis the cause of missed diagnosis of cerebral infarction in the schizophrenia patients and explore the measures to reduce misdiagnosis.Methods：127 cases of schizophrenia in our hospital from March 2009 to March 2010 were studied retrospectively,by using CT and（or） MIR to check cerebral infarction.Results：11 cases（8.66%） of cerebral infarction among 127 patients with schizophrenia were found by CT and（or） MRI diagnosed;most of the diagnosed patients had significant psychiatric symptoms,while there was an relative between disease and infarction area.The patients were improved after the treatment corresponding to cerebral infarction.Conclusion：Schizophrenia associated with cerebral infarction is clinically rare,especially those with significant psychiatric symptoms,but easily missed when obvious signs of clinical physicians were absent.The doctor should pay attention to the diagnosis of mental disorder,so as not to delay the timing of treatment.
