红细胞参数对轻型β-地中海贫血与缺铁性贫血鉴别诊断的价值

Values of red blood cell indicators in differential diagnosis of beta-thalassemia minor and iron-deficiency anemia

目的探讨红细胞参数在轻型β-地中海贫血(简称β-地贫)和缺铁性贫血鉴别诊断中的价值。方法对210例轻型β-地贫患者、100例缺铁性贫血患者和100例健康成人血细胞进行检测分析,并对红细胞参数中RBC、Hb、平均红细胞体积(MCV)、平均红细胞血红蛋白含量(MCH)、红细胞体积分布宽度(RDW)、RBC/MCV等指标进行比较。结果轻型β-地贫组RBC、RBC/MCV高于缺铁性贫血组和健康对照组,缺铁性贫血组RBC、Hb显著低于健康对照组和轻型β-地贫组,RDW显著增高。两贫血组MCV和MCH均显著低于对照组。将RBC、Hb、MCV、MCH、RDW、RBC/MCV同时作为筛查指标评价轻型β-地贫和缺铁性贫血的灵敏度分别为93.6%、97.7%,符合率分别为77.2%、80.0%。结论同时检测RBC、Hb、MCV、MCH、RDW、RBC/MCV等指标有助于鉴别诊断轻型β-地贫和缺铁性贫血,是筛查轻型β-地贫经济、有效的方法。

Objective To explore the values of red blood cell indicators in differential diagnosis of beta-thalassemia minor and iron-deficiency anemia（IDA）.Methods The routine hematological examination was performed with full automatic hemocyte analyzer and the parameters including RBC,Hb,MCV,MCH,RDW and RBC/MCV were analysed in 210 thalassemia patients,100 iron-deficiency anemia patients and 100 normal subjects.Results In beta-thalassemia minor group,the total number of RBC and RBC/MCV were significant higher than those in the nomal group.The total number of RBC,Hb in IDA group were lower,and RDW was significant higher than that in the other two groups.The specificity and accuracy of RBC,Hb,MCV,MCH,RDW and RBC/MCV in screening of beta-thalassemia minor were 93.6% and 77.2%,which of IDA were 97.7% and 80%.Conclusion The measurement of RBC,Hb,MCV,MCH,RDW and RBC/MCV is contributive in differential diagnosis of beta-thalassemia minor and iron-deficiency anemia and also is an economical and efficient method in screening of beta-thalassemia minor.
