2型糖尿病血管病变中医证候积分与细胞免疫状态的相关性

Correlation between TCM Syndrome Scores and Celluar Immune Status of Type Ⅱ Diabetes Patients with Angiopathy

目的探讨2型糖尿病下肢血管病变气阴两虚兼血瘀证患者中医证候积分与细胞免疫状态的关系。方法根据中医证候积分分级标准,将符合纳入标准的74例2型糖尿病下肢血管病变患者分为轻度、中度、重度3组,另设10例健康志愿者为对照组,观察各组CD 4、CD 4%、CD 8、CD 8%、CD 4/CD 8的变化情况并分析CD 4、CD 8与中医证候积分的相关性。结果 3组中轻、重度组间不同年龄段比较其差异有统计学意义(P<0.05),轻、重度组间及中、重度组间诊断糖尿病时间比较其差异有统计学意义(P<0.05);3组间CD 4、CD 8、CD 4%比较其差异有统计学意义(P<0.05);CD 4与气虚证积分、血瘀证积分、诊断糖尿病时间呈负相关(P<0.05),与阴虚证积分、患者年龄无相关性(P>0.05);CD 8与阴虚证积分、血瘀证积分呈正相关(P<0.05),与气虚证积分、患者年龄、诊断糖尿病时间无相关性(P>0.05)。结论 CD 4细胞数量的减少和CD 8细胞数量的增加是2型糖尿病下肢血管病变气阴两虚兼血瘀证患者机体细胞免疫状态的重要特征;CD 4细胞数量的减少、CD 8细胞数量的增加与中医气虚血瘀证相关,气、阴的失调及血瘀证的程度影响机体细胞免疫的状态。

Objective To study the correlation between TCM syndrome scores and celluar immune status of the type Ⅱ diabetes patients with angiopathy of lower limbs who were differentiated as qi and yin deficiency accompanied with blood stasis syndrome.Methods Based on the grading criteria of TCM syndrome scores,the 74 patients enrolled were divided into mild,medium,and severe groups and 10 healthy volunteers were taken as the control group.The changes in CD4,CD4%,CD8,CD8%,and CD4/CD8 were observed and the correlation between CD4,CD8 and TCM syndrome scores was analyzed.Results Of the three groups, there was statistical difference in age between the mild and severe groups（P0.05）,and there was statistical difference in the time of diagnosing diabetes between the mild and severe groups and between the medium and severe groups（P0.05）.There was statistical difference in CD4,CD8,and CD4% among the 3 groups（P0.05）.CD4 was negatively correlated with the scores of qi deficiency syndrome,blood stasis syndrome,and the time of diagnosing diabetes（P0.05）,but was not related with the scores of yin deficiency syndrome,and the age of patients.CD8 was positively correlated with the scores of yin deficiency syndrome and blood stasis syndrome（P0.05）,but was not related with the scores of qi deficiency syndrome,age of patients,and the time of diagnosing diabetes（P0.05）.Conclusion The CD4 decrease and CD8 increase are the important characteristics of celluar immune status of type Ⅱ diabetes patients with angiopathy of lower limbs in qi and yin deficiency accompanied with blood stasis syndrome,and they are related with the qi deficiency and blood stasis.Qi and yin dysfunction and the severity of blood stasis exert influence on the celluar immune status of the body.
