慢性乙型肝炎e抗原阴性患者肝郁脾虚证与HBV基因型和基因变异类型的相关性

Correlation of Liver Stagnation and Spleen Deficiency Syndrome with HBV Genetype and Genovariation in Chronic Hepatitis B Patients of HBeAg in Negative

目的探讨慢性乙型肝炎e抗原(HB eA g)阴性患者乙型肝炎病毒(HBV)基因型和基因变异类型与中医肝郁脾虚证的相关性。方法选取76例HB eA g阴性且被辨证为肝郁脾虚型的慢性乙肝患者,检测其HBV基因型和BCP区、前C区变异的情况,并同湿热中阻型患者做比较,同时结合肝郁脾虚型证型积分情况进行相关性分析。结果肝郁脾虚型与湿热中阻型患者相同位点的变异率差异有统计学意义(P<0.05);两组证型患者在HBV基因型的分布上差异有统计学意义(P<0.05);各基因变异类型占各HBV基因型的比例也不相同(P<0.05);肝郁脾虚型患者各基因型及各基因变异类型肝郁脾虚证候计分比较,差异亦有统计学意义(P<0.05)。结论 C基因型和前C变异在慢性乙型肝炎HB eA g阴性肝郁脾虚型患者中常见,HBV基因型和基因变异与中医证型均有相关性。

Objective To study the correlation of the HBV genetype and genovariation with the syndrome type of liver stagnation and spleen deficiency of the chronic hepatitis B patients with HBeAg in negative.Methods Seventy-six chronic hepatitis B patients with HBeAg in negative and diagnosed as liver stagnation spleen deficiency syndrome in TCM were detected,their HBV genotype and genovariation in BCP and pre-C areas were compared with those of the patients diagnosed as damp-heat obstructing the middle jiao,at the same time,a correlation analysis was done with the syndrome score of liver stagnation spleen deficiency taken into consideration.Results The variation rate of the patients with liver stagnation spleen deficiency and those with damp-heat obstructing the middle jiao was statistically different（P0.05）,the HBV genotype distribution of patients of both syndromes was statistically different（P0.05）,the proportion of each genovariation type in the HBV genotype was also different（P0.05）,and the comparison of syndrome scores of patients with liver stagnation spleen deficiency in all genotypes and genovariations also showed a difference in statistics（P0.05）.Conclusion The C genotype and pre-C area genovariation are commonly seen in the chronic hepatitis B patients with HBeAg in negative in TCM liver stagnation spleen deficiency syndrome.Both the HBV genotype and genovariation are correlated with the types of TCM syndromes.
