3D DCE MRA for Diagnosing Aortic Dissection

摘要 目的:探讨3DDCE MRA 对主动脉夹层诊断的价值及其检查技术。材料和方法:共作主动脉夹层28 例3DDCEMRA 检查。采用3DFSGR,团注Gd - DTPA20ml。屏气扫描时间16 ~22 秒。经工作站进行MIP 和MPR 重建。结果:分析28 例主动脉夹层3DDCE MRA 表现,发现3DDCE MRA 对主动脉夹层的上下范围,内膜片、破口、真假两腔以及分支受累情况均能满意显示。比较MIP,MPR 及原始图像对内膜片的显示和MIP 及MPR 对分支受累的显示,发现MPR 及原始图像对内膜片的显示优于MIP。MPR 对分支受累的显示优于MIP。结论:在常规MRI 检查基础上,辅以3DDCE MRA 技术,将大大提高MR 对主动脉夹层的显示和诊断能力。 Purpose:To investigate and evaluate the value,technique and findings of 3D DCE MRA for diagnosing aortic dissection.Materials and Methods:28 cases of 3D DCE MRA for diagnosing aortic dissection were performed.3D FSPGR was used during bolus injection of Gd-DTPA 20ml.Acquisition time took 16-22 sec with breath-hold.MIP and MPR were reconstructed from source images on workstation.Results:The imaging datas of 28 cases of 3D DCE MRA for diagnosing aortic dissection were analysed.The extent,intimal flap, entry tear, double lumen and involvement of aortic branches of aortic dissections were visualized satisfactorily.The demonstrations of intimal flaps among MIP,MPR and source images were compared with one another showing MPR and source images were better than MIP.The demonstrations of involvements of branches between MIP and MPR were also compared with each other showing the latter was better.Conclusion:Conventional MRI associated with 3D DCE MRA Will greatly improve the capabilities of MR in displaying and diagnosing aortic dissection.
