超声检查在诊断闭合性肝脏损伤中的临床价值

Clinical value of ultrasonography in the diagnosis of closed hepatic trauma

目的探讨超声检查在诊断闭合性肝脏损伤中的临床价值。方法回顾总结分析92例闭合性肝脏损伤患者，并对超声图像诊断与外科手术所见进行对照。结果92例中，肝包膜下破裂12例（12／12），肝中央型破裂17例（17／17），肝破裂63例（61／63），诊断符合率：96．83％。合并脾破裂25例、肾挫伤11例、胰腺挫伤7例、肠破裂2例。超声图像诊断与外科手术诊断符合率达96．83％。结论超声检查方法简便、经济实惠、快速有效、无创伤、可以重复，是诊断闭合性肝脏损伤的首选方法，对临床诊断及治疗方案的制定具有重要的指导价值。

Objective To investigate the application of ultrasonography in the diagnosis of closed hepatic trauma. Methods To review and summarize the collected data for 92 cases of diagnosis by ultrasonography for closed hepatic trauma, The results of conventional ultrasonography were compared with surgery findings. Results Among the 92 cases of closed hepatic trauma, 12 case of subcap sular rupture, 17 cases of central rupture, 63 cases of true rupture, 19 cases of true hepatic rupture incorporated with spleen rupture, 11 cases contusion of kidney, 7 cases contusion of pancreas. Conclusion That ultrasonography in the diagnosis of closed hepatic trauma shows a number of advantages such as convenient, low cost, and effective thusshould be considered as the top priority.
