影响部队不同类型病毒性肝炎患者住院费用的因素分析

Analysis of Factors Affecting Medical Care Costs of Hepatitis Cases among Military Population

目的 确定部队病毒性肝炎患者住院费用及其影响因素。方法 资料来自全军卫生统计信息中心病案首页数据库,应用SPSS统计软件包处理,统计方法有方差分析、多元逐步回归分析等。结果 部队病毒性肝炎患者住院费用中,药费所占比重较大,达54.39％,床位费占21.04％,治疗费仅占17.66％。单因素分析结果表明:住院天数对病毒性肝炎住院费用有显著影响。入院情况及治疗结果等相关因素对病毒性肝炎住院费用均有明显影响(P<0.01),另外,年龄、婚姻状况、不同职级别及军兵种等因素也有一定影响(P<0.01)。逐步回归结果显示:影响患者住院费用的因素按作用大小依次为:住院天数、患者年龄、一级护理、年度、是否手术、治疗结果、入院情况、有无并发症、是否发生院内感染、军种、性别、病原类型等。结论 部队乙肝发病率高,住院天数是影响军队病毒性肝炎患者住院费用的主要原因。同时,年龄、一级护理、年度等因素对住院费用均有不同程度的影响。

Objective To determine the factors affecting medical care costs of hepatitis cases among military population. Methods Raw data were collected from the headlines of inpatient medical records in the data base of PLA Health Statistical Information Center. processed by SPSS and analysed by analysis of variance, stepwise regression etc. Results Among medical care costs of hepatitis cases in military population, medication was 59. 75%, and ac-counted for a higher proportion, which exceeded 50%, ward charge about 20% and treatment only 17% ; The results of single variable analysis indicated the following: Length of stay affected the medical care costs of hepatitis patients signicantly(P<0. 01). The factors such as admission state and the results of treatment affected the medical care costs of patients obviously(p <0. 01). In addition, age, marriage status. official position and armed services influ-enced the costs statistically (P<0. 01). Stepwise regression analysis showed that length of stay, age, nurse, admission year, surgical operation, the results of treatment, complication, infection, armed services, sex and type of hepatitis affected the medical care costs. Conclusion High incidence of HB in military population and days of hospital stay were two major factors affecting the costs, and the latter was related to the costs positively. At the same time, age, the state of the case results of the treatment affected the medical care costs in different degrees.
