急性脑出血后应激性高血糖与预后的关系

The correlation of the stress hyperglycemia and prognosis after intracerebral hemorrhage

目的:探讨急性脑出血后应激性高血糖与预后的相关性。方法:238例无糖尿病史急性脑出血患者按入院时血糖范围分为3组,第1组血糖>7.0mmol/L,第2组6.1～7.0mmol/L,第3组<6.1mmol/L,对其进行χ2检验。结果:第1组患者有126例,神经缺损程度评分ADL≤20分9例,21～60分46例,>60分71例;第2组有患者89例,ADL≤20分17例,21～60分33例,>60分39例;第3组有患者23例,ADL≤20分13例,21～60分8例,>60分2例。神经缺损程度分布情况,经过χ2检验,P<0.05,差异有统计学意义。结论:应激性血糖升高与预后有正相关性,血糖越高,致残率、病死率越高。应激性高血糖明显影响脑出血患者的预后,积极控制血糖水平有助于改善患者预后。

Objective：To study the interrelation between stress hyperglycemia and prognosis after intracerebral hemorrhage（ICH）.Methods：238 patients without diabetes mellitus in patients with acute cerebral hemorrhage at the time of ad mission blood glucose were divided into 3 groups,the first group blood glucose 7.0 mmol/L,the second group 6.1 mmol/L, 7.0 mmol/L,the third group 6.1 mmol/L,taken χ2 test.Results：The first group,there were 126 patients;neurological deficit scores（ADL）≤20 were 9 patients,ADL betwen 60～21 were 46 patients,60 were 71 patients;the second group there were 89 patients;neurological deficit scores（ADL）≤20 were 17 patients,ADL betwen 60～21 were 33 patients,60 were 39 patients;and in third group there were 23 patients ;neurological deficit scores（ADL） ≤20 was 13 patients,ADL betwen 60～21 were 8 patients,60 were 2 patients.Taken χ2 test,the difference was significant.Conclusion：Prognosis after intracerebral hemorrhage is correlated with stress hyperglycemia.To control blood sugar level contributes to prognosis after intracerebral hemorrhage.
