WU多瘤病毒在儿童急性呼吸道感染中的作用 被引量:8

Role of WU polyomavirus in children with acute respiratory tract infections

摘要 目的了解急性呼吸道感染儿童中WU多瘤病毒(WUPyV)的感染情况及其作用。方法采集天津市儿童医院急性下呼吸道感染住院儿童的鼻咽吸取物(NPA)标本174份,急性上呼吸道感染的咽拭子标本68份以及无呼吸道症状的对照咽拭子标本43份,用PCR法进行WUPyV基因检测,对NPA阳性标本采用PCR和多重PCR同时检测其他常见呼吸道病毒。结果174份NPA标本中共28份检出WUPyV核酸,占16.1%。患者平均年龄为11.7个月(12 d^39个月),其中≤6个月10例(35.7%),6个月~1岁10例(35.7%),1~2岁7例(25.0%),2~5岁1例(3.6%);男19例(67.9%),女9例(32.1%);与其他呼吸道病毒混合感染20例,占71.4%,50%为两种病毒混合感染,混合感染率最高的为RSVB(32.1%),其次为HboV(21.4%)和Rhino、PIV3(均为14.3%)。检查68例急性上呼吸道感染者的咽拭子标本,3例为WUPyV阳性,其中2例患儿为3岁,1例2岁;女2例,男1例。43例无呼吸道症状者的咽拭子标本均为WUPyV阴性。结论急性呼吸道感染儿童中存在WUPyV感染,下呼吸道感染患儿中检出率高于上呼吸道感染患儿。无呼吸道症状对照儿童中未检出WUPyV。WUPyV可能是儿童急性呼吸道感染中的重要病原之一,但其是否为独立致病因子还需进一步研究。 Objective To investigate the potential role of WU polyomavirus in respiratory disease,the frequency of its detection and its epidemiological characteristics were determined in children with and without respiratory infections.Methods Nasopharyngeal aspirates(NPA) were collected from 174 hospitalized pediatric patients from Mar.to Apr.in 2008 and from Sep.2008 to Feb.2009.VP2 gene fragments of WUPyV were first examined by PCR amplification and then verified by PCR amplification of LTAg gene fragments.All WUPyV-positive NPA samples were examined by PCR and multi-PCR for other respiratory viruses.Throat swabs were collected from 68 children with an upper respiratory tract infection(URTI) and 43 children with no signs of a respiratory tract infection.Results Of 174 hospitalized pediatric patients,28(16.1%) were positive for WUPyV.The mean age of children infected with WUPyV was 11.7 months(from 12 days to 39 months);10 cases(35.7%) were ≤6 months of age,10(35.7%) were 6 months-1 year of age,7(25%) were 1-2 years of age,and 1(3.6%) was 2-5 years of age.Of the children,19(67.9%) were male and 9(32.1%) were female.Twenty patients(71.4%) had a combined infected with another respiratory virus.The other virus most commonly detected was RSV B(32.1%),followed by HBoV(21.4%) and Rhino and PIV3(14.3%).Of 68 throat swabs from children with an URTI,3 were WUPyV-positive.Two patients were 2 years old and 1 was 1 year old;2 were female and 1 was male.No WUPyV was detected in 43 samples collected from asymptomatic control patients.Conclusion A high prevalence of WUPyV was detected in hospitalized children with respiratory infections in Tianjin,China.A higher percentage of WUPyV was obtained from hospitalized children with an LRTI than with a URTI.WUPyV was not found in asymptomatic children.WUPyV may be an important agent in children with respiratory tract infections.
