TopoⅡ、P170、GST-pi蛋白在胃癌中表达与临床病理及预后的关系 被引量:1

摘要 [目的]探讨胃癌中P-糖蛋白(P170)、DNA拓扑异构酶Ⅱ(TopoⅡ)和谷胱苷肽-S转移酶(GST-pi)的表达和胃癌的临床病理特征及其预后的关系及意义。[方法]免疫组化检测160例胃癌组织中的P170、TopoⅡ及GST-pi的表达,收集临床病理资料,随访患者的生存时间。分析各因子表达、临床病理资料和预后的关系。[结果]P170和GST-pi的表达呈正相关(P<0.01),P170表达与肿瘤直径、淋巴结转移、远处转移及TNM分期呈正相关(P<0.05)。GST-pi表达与远处转移及TNM分期呈正相关(P<0.05)。TopoⅡ的表达与侵袭深度、远处转移及TNM分期呈正相关(P<0.05)。单因素分析发现患者年龄、肿瘤直径、肿瘤部位、病理分级、侵袭深度、淋巴转移、远处转移、TNM分期、TopoⅡ、P170、GST-pi均与预后有关(P<0.05)。Cox风险模型多因素分析发现肿瘤部位、病理分级、侵袭深度、淋巴转移、GST-pi是影响胃癌预后的独立因素(P<0.05)。[结论]TopoⅡ、P170、GST-pi均与胃癌的某些临床病理特征呈一定的相关性,肿瘤部位、病理分级、侵袭深度、淋巴转移、GST-pi是影响胃癌预后的独立因素。 [Objective] To evaluate the expression of P-glycoprotein(P170),Topoisomerase Ⅱ(TopoⅡ) and glutathione S-transferase π(GST-pi) in gastric carcinoma,their possible relationship with clinicopathologic parameters of tumors,and their prognostic significance.[Methods] The expression of P170,TopoⅡ and GST-pi in 160 patients with gastric carcinoma was detected by immunohistochemistry and the patients were followed up for more than three years.The relationship between the expression of P170,TopoⅡ,GST-pi and the clinicopathologic parameters were analyzed.The relationship between these parameters and prognosis of patients were evaluated.[Results] There was a positive correlation between the expression of P170 and GST-pi.The intensity of P170 was positively related to the diameter of tumor,lymphatic metastasis,distant metastasis and TNM grade(P0.05).The expression of GST-pi was associated to the pathological grade,distant metastasis and TNM grade(P0.05).The expression strength of TopoⅡwas positively related to the pathological grade,lymphatic metastasis,depth of invasion and TNM grade(P0.05).In univariate analysis,age,tumor diameter,tumor site,histological type,depth infiltration,lymphatic metastasis,distance metastasis,TNM grade and the expression of TopoⅡ,P170,GST-pi were related to the prognosis of gastric cancer(P0.05).Multivariate Cox model analysis showed that the tumor site,histological type,depth infiltration,lymphatic metastasis and GST-pi were the independent prognostic factors for patients(P0.05).[Conclusion] The expression of TopoⅡ,P170,GST-pi are related to some of the histopathological features,and some of these factors can be regarded as prognostic factors in gastric carcinoma.
