单侧唇裂继发鼻畸形综合整复术

A modified technique for the repair of secondary unilateral cleft nasal deformity

目的 探讨单侧唇裂继发鼻畸形理想的修复方法.方法 碟形切口显露鼻翼软骨,切开鼻底,松解并复位错位的口轮匝肌,松解并收拢患侧鼻翼外侧脚,提高并固定患侧鼻翼内侧脚,复位偏离的鼻中隔.结果 本组共43例患者,2例术后假体移位.35例获随访6个月,患侧鼻翼塌陷、鼻孔不对称、鼻尖不对称和鼻中隔偏斜等畸形得到明显改善,鼻底部洞状缺损及上唇瘢痕亦得到较大改善.结论 对鼻翼软骨、鼻中隔畸形的修复能使单侧唇裂继发鼻畸形获得良好的修复效果.

Objective To investigate the effect of a modified technique for reparing secondary unilateral cleft nasal deformity. Methods Nasal alar cartilage was explosed through the dish-like incision, and base of naris was cut to loosening and replacing the orbicnlaris oris, releasing and moving close the lateral crus of ala nais, liftig and fixing the medial crus of ala nais and replacing the deviated nasal septum. Results Satisfactory results were achieved in all 43 eases Except for 2 cases with the graft malposition. After 6 months following up on 35 eases, all cases were satisfied with the apparent improve,nent of these deformities, such as collapsed nasal alar, asymmetrical naris and tip and deviated nasal septum. The hole-like defect of basis nasi and upper lip scar were also improved comparatively. Conclusion This modified technique with nasal septum and alar cartilage reposition is a good option for the repair of secondary unilateral cleft nasal deformity.
