头皮旋转瓣修复钛网外露感染创面 被引量:12

Repair of infectious scalp defects with titanium mesh exposure by scalp rotation flap

摘要 目的 探讨头皮旋转瓣在头部钛网外露感染创面修复中的应用.方法 本组患者共12例,男性4例,女性8例.创面大小0.5 cm×0.5 cm~2.0 cm×5.0 cm.早期处理创面,控制感染.术中根据钛网外露的部位、大小、原手术切口的走向,设计单个头皮旋转瓣修复头部钛网外露创面.结果 术后所有皮瓣均完全成活,1例修复后因多次放疗再次出现直径约1.0 cm钛网外露,于原旋转瓣内设计一小型旋转瓣,成功修复创面,余者均未再次出现钛网外露.结论 运用带有丰富血运的头皮旋转瓣,可在不去除钛网的情况下有效地修复钛网外露感染创面. Objective To explore the application of scalp rotation flap in reconstruction of infectious scalp defect with titanium mesh exposure. Methods Twelve patients were treated in this group including 4 male and 8 female. The defective size ranged from 2.0 cm× 5.0 cm to O. 5 cm × 0.5 cm. After local treatment and infection control, single scalp rotation flap was designed according to the position, size of exposed titanium mesh and the direction of originally operative incision to repair the defect. Results All scalp flaps survived except for one case case 1.0 cm titanium mesh exposure again because of multi-times radiotherapy which was covered successfully with a mini rotation flap in original rotation flap. Conclusion The application of scalp rotation flap with affluent vessel can repair the infectious scalp defects efficiently without removal of the titanium mesh.
