以胸廓内血管为蒂的肋骨复合组织瓣修复口腔下颌骨缺损的研究

摘要 目的 探讨以胸廓内血管为蒂的肋骨复合组织瓣,修复口腔下颌骨缺损的可行性及其应用价值.方法 回顾性分析13例以胸廓内血管为蒂的肋骨复合组织瓣,修复口腔下颌骨缺损的患者资料,记录以胸廓内血管为蒂的肋骨复合组织瓣切取要点、骨肌皮瓣,术后观察判断指标.结果 13例以胸廓内血管为蒂的肋骨复合组织瓣全部成活,10例患者于术后6个月后行义齿修复,功能及外形满意.曲面断层片示移植的肋骨无骨吸收.结论 对于下颌骨复合组织缺损,尤其是伴髁突缺损者,以胸廓内血管为蒂的肋骨复合组织瓣修复是一种较理想的方法. Objective To study the feasibility and the value of repair of oral mandibular defects with rib composite flap pedicled with internal thoracic vessels in the basic level hospitals. Methods The clinical materials in 13 cases uith mandibular defects which were repaired with rib composite flap pedicled with internal thoraeie vessels were analyed retrospectively. The cutting main points of rib composite flap and the postoperative parameters of osseous inyocutaneous flap were recorded and analyzed. Results All 13 rib composite flaps pedicled with internal thoracic vessels survived well and 10 cases were repaired with denture at 6 months postoperatively. The function and shape of the mandible were excellent. Bone resorption of transplanted rib were not observed in panoramic pantoinogram. Conclusion In the basic level hospitals, rib composite flap pedicled with internal thoracic vessels is an ideal method for the repair of mandibular composite tissue defects, especially for the mandibular defects accompanied with condyle defects.
