内镜下射频治疗术联合氩离子凝固术在贲门-胃底癌治疗中的应用

Treatment of tumor of cardia-gastric bottom with radiofrequency and argon plasma coagulation under endoscopy

目的贲门-胃底癌经内镜检查证实时多属晚期癌,多数合并有多脏器转移,手术难以切除,文中探讨射频与氩离子凝固术联合应用在贲门-胃底癌治疗中的疗效。方法随机选取2002年10月至2008年5月37例贲门-胃底癌Ⅳ级狭窄的患者,使用射频及氩离子凝固术使贲门通畅,防治慢性失血。结果 37例患者有26例经反复多次治疗,梗阻解除,胃底肿瘤生长趋缓。结论内镜下联合射频与氩离子凝固术治疗贲门-胃底癌显著改善了患者的生存质量。

Objective Cardia-gastric bottom tumors,when confirmed by endoscopes,are often advanced,complicating multitude organ metabasis and hard to resect by operations.This paper is to study the application of radiofrequency ablation（RFA） and argon plasma coagulation（APC） on treating severe strictuer of cardia-gastric bottom tumor under endoscopy and deferring growth of tumor.Methods Thirty-seven patients with severe carcinous cardia-stomach stricture（Ⅳ grade） were treated by RFA and APC to make cardia overpasses,to control the growth of gastric bottom tumor and to prevent and cure chronic bleeding.Results There are twenty six sufferers of thirty seven cases treated by RFA and APC frequently and released from cardiac obstruction,tumor of cardia-gastric bottom growth changed tendency slowness.Conclusion Combined treatment of RFA and APC on severe carcinous stricture of cardia-gastric bottom tumor may prolong sufferers′ life and improved their life qualities.
