摘要
目的探讨白细胞介素18（IL-18）、白细胞介素18结合蛋白（IL-18BP）和干扰素γ（IFN-γ）在乙型肝炎肝硬化患者中血清水平的变化及其与病情的关系。方法用ELISA法检测60例乙型肝炎肝硬化患者（Child—PughA级、Child—PughB级、Child—PughC级各20例）、20例正常对照者血清IL-18，IL-18BP，IFN-γ水平，用全自动生化分析仪检测上述4组的白蛋白（ALB）、总胆红素（TBIL）水平，用全自动血凝仪检测4组的凝血酶原活动度（PTA）水平。结果乙型肝炎肝硬化患者血清IL-18，IL-18BP，IFN-γ水平均高于正常对照组（P〈0．01），且IL-18，IFN-γ水平随着肝硬化Child—Pugh评分上升而进行性升高，而IL-18BP则在Child—PughB级最高。IL-18，IFN-γ，IL-18BP与TBIL成正相关，与ALB，PTA成负相关（P〈0．05）。结论IL-18，IL-18BP，IFN-γ参与了宿主对HBV的免疫应答，三者之间存在相互关系，检测三者的水平有助于判断乙型肝炎肝硬化患者的病情。
Objective To study the relationship between the serum level of interleukin-18（IL-18） ,interleukin -18 binding protein（ IL-18BP） and interferon-3,（IFN-γ） in patients with Hepatitis B cirrhosis and disease severity. Methods Serum level of IL-18,IL-18BP and IFN-γ were detected by ELISA in 60 patients with Hepatitis B cirrhosis （ Chi I d-Pugh A ,20 cases ; Child-Pugh B ,20 cases ; Child-Pugh C, 20 cases） and 20 healthy people. The serum level of ALB and TBIL in the four groups were tested by Automatic Biochemical Analyzer. And their serum contents of PTA were detected by Automatic Coagulometer. Results The serum level of IL-18, IL-18BP, IFN-γ was significantly higher in patients with Hepatitis B cirrhosis than those in control group （ P 〈 0.01 ） , and the expression of IL- 18, IFN-γwas positively correlated with the Child-Pugh grading,while the expression level of IL-IgBP in Child-Pugh B was ＇the highest. The serum level of IL-18 ,IL-18BP, IFN-3, was correlated with serum TBIL and inversely correlated with that of ALB and PTA. Conclusion IL-18 IIL-18BP and IFN-γ involved in the immune response of the patients with HBV infection. Detecting the level of the three index are helpful in the evaluation of disease severity in patients with Hepatitis B cirrhosis.
