摘要
目的探讨广西地区未感染艾滋病病毒(HIV)婴儿T淋巴细胞及亚群的基本特征,为初步排除并辅助诊断HIV阳性产妇所生婴儿是否感染HIV提供依据。方法收集68例未感染HIV婴儿(未感染组)和13例感染HIV婴儿(对照组)的抗凝全血,用四色荧光流式细胞术检测外周血CD3+、CD4+、CD8+T淋巴细胞绝对数及其百分比,并计算CD4/CD8比值;用Nuclisens EasyQ方法检测血浆HIV-1病毒载量。结果 (1)未感染组外周血CD3+、CD4+、CD8+T淋巴细胞绝对数(细胞/mm3)的中位数分别为4 5702、8181、410;CD3、CD4、CD8的百分比中位数分别为61.42%、39.39%、18.40%;CD4/CD8比值的中位数为2.25;在未感染组中,男性婴儿与女性婴儿的T淋巴细胞及亚群的比较,各检测指标差异没有统计学意义(P>0.05);血浆HIV-1病毒载量均为未检测出水平。(2)对照组外周血CD3+、CD4+、CD8+T淋巴细胞绝对数的中位数分别为4 930、1 4603、326(细胞/mm3);CD3、CD4、CD8百分比的中位数分别为69.95%、13.77%、54.69%;CD4/CD8比值的中位数为0.25;血浆HIV-1病毒载量的中位数为800 000(拷贝/ml)。(3)除CD3+T淋巴细胞绝对数之外,未感染组与对照组的CD4+、CD8+T淋巴细胞绝对数、CD3、CD4、CD8百分比及CD4/CD8比值的比较,差异有统计学意义(P<0.05)。结论在资源有限的情况下,参考婴儿T淋巴细胞及亚群的异常变化,可初步判断HIV阳性产妇所生婴儿是否感染HIV,T淋巴细胞及亚群的检测,可以作为一种辅助诊断HIV感染的手段。
Objective To explore the basic characteristics of T lymphocytes and their subsets among the infants un-infected with HIV,and provide evidence for diagnosing whether the infants born to HIV-positive mothers are infected with HIV.Methods The anticoagulant whole blood samples were collected from 68 cases of HIV un-infected infants(un-infected group) and 13 cases of HIV infected infants(control group).The absolute values of CD3+,CD+4 and CD+8 T lymphocytes in peripheral blood were detected by using four-color fluorescence flow cytometry,and the percentages of CD3+,CD+4 and CD+8T lymphocytes and the ratio of CD4/CD8 were calculated.HIV-1 viral load of plasma was detected by EasyQ.Results In the un-infected group,the median absolute values(cells/mm3) of CD3+,CD+4 and CD+8T lymphocytes were 4570,2818 and 1410 respectively;the median CD3%,CD4%,and CD8% were 61.42%,39.39% and 18.40% respectively;the median CD4/CD8 was 2.25.In the un-infected group,the characteristics of T lymphocytes and subsets in the male infants were not different from those in the female infants.The level of HIV-1 viral load in the un-infected group was not detected.In the control group,the median absolute values(cells/mm3) of CD3+,CD+4 and CD+8T lymphocytes were 4930,1460 and 3326 respectively;the median CD3%,CD4% and CD8% were 69.95%,13.77% and 54.69% respectively;the median ratio of CD4/CD8 was 0.25.The median levels of HIV-1 viral load in the control group was 800000(copies/ml).Except for the absolute values of CD3+T lymphocytes,the absolute values of CD+4 and CD+8T lymphocytes,CD3%,CD4% and CD8% and the ratio of CD4/CD8 in the un-infected group had significant difference,as compared with those in the infected group(P0.05).Conclusion In the limited resources,the abnormal T lymphocytes and subsets in the infants born to HIV-positive mothers may be used as a reference to judge preliminarily whether he or she is infected with HIV.Consequently,the detection of T lymphocytes and subsets can serve as a complementary tool to diagnose HIV infection.
