为了加强脑血管意外患者康复期的健康管理,提高患者的生活质量,我们建立了医院-社区-家庭三位一体的健康管理新模式,使脑血管意外患者对医院的满意度和自身的生活质量得到提高,有效地降低了疾病的复发和医疗支出。

Objective：To improve the cerebrovascular accident patients＇life quanlity and their healthcare treatment effect in healing period,we settled to establish a new healthcare management system,which we called a＂hospital-community-family＂ three-in-one system.This healthcare management system suceeded in enhancing the cerebrovascular accident patients＇ life quanlity and their patient-satisfaction;it also mamaged to cut down the patients＇medical expensure and to rudeuce these disease recrudescence rate effectively.
