重组人红细胞生成素(EPO)在口腔颌面外科手术自身输血中的应用

THE APPLICATION OF HUMAN RECOMBINANT ERRTHROPOIENTIN IN THE ORAL MAXILLOFACIAL SURGERY WITH AUTOLOGOUS BLOOD TRANSFUSION

目的:观察和比较重组人红细胞生成素(EPO)、铁剂在口腔颌面外科手术贮血式自身输血中对贮血后贫血的影响。方法:23例颌骨整形病人,随机分二组。铁剂组:贮血前一周至术前口服硫酸亚铁300mg,每日三次;铁剂合并EPO组:铁剂服用方法二组相同,贮血后隔日静注EPO6000U,每周18000U至术前。观察二组贮血前、手术前RBC、Hb、HCT的改变,计算贮血后至手术前Hb增加值并进行对比。结果:EPO合并铁剂组手术前RBC、Hb、HCT下降幅度小于铁剂组,两组差别有显著意义(P<0.05)。Hb增加值大于铁剂组,两组间有显著性差异(P<0.01)。结论:EPO合并铁剂能有效治疗和预防贮血后贫血。

Objective: To observe and compare the effect of Anemia which predeposited autologous blood transfusion with human recombinant erythropoietin and iron sulphant in oral maxillofacial surgery. Methods: Twenty three cases were randomly dividede into 2 groups Iron sulphant group had nine patients. Before a week predeposited blood till preoperative each poatient received iron sulphant 300mg, po, tid. Combine EPO with iron sulphant group had fourteen poatients, the administration of iron sulphant were the same of two groups. After predeposited blood, the EPO 6000 unit were intravenous every other day till preoperative. Rbc, Hb, Hct were observed before predepoitedd dblood and preoperative. Results: Preoperative, RBC, HB, Hct in iron sulphant group were significntly lower than combine EPO with iron sulphant group ( P <0.05). Hb in combine EPO with iron sulphant group were increased significantly higher than iron sulphant group ( P <0.05). Conclusion: Anemia that after predeposited autologous blood can be relieved by EPO with iron sulphant.
