超声心动图评价左房容积与左室舒张功能的关系及对心血管事件的预测 被引量:9

摘要 目的探讨左房容积与左室舒张功能之间的关系,及其对未来心血管事件的预测价值。方法追踪观察研究组(左房进行性增大,排除风心病、肺心病、高心病、心肌炎、心肌病、糖尿病及甲状腺疾病等器质性心脏病患者)120例,对照组(左房大小正常的无症状患者)120例,两组均在窦性心率下测量左房前后径(LAd)、左房长轴径(Lx),左房短轴径(Sx),舒张早期血流峰值速度(Ve),舒张晚期血流峰值速度(Va),舒张早期E峰减速时间(DT),等容舒张时间(IVRT),心房收缩期肺静脉逆向血流(Var)。并观察其预后表现。结果研究组与对照组间E/A<1,百分率有显著差异;两组间左房容积指数有显著差异;随左房容积指数增加,舒张功能恶化;研究组中120例患者在随后3年内发生房颤45例,充血性心率衰竭23例,脑卒中11例,死亡6例;对照组中120例患者未发现不良心血管事件。结论超声心动图对左房容积变化的观察对判断左室舒张功能及预测未来心血管事件具有重要意义。 Objective To explore the relationship between left atrial volume and left ventricular diastolic function and the value of left atrial volume to prediction cardiovascular events.Methods To evaluate the left atrial volume and left ventricular diastolic function with echocardiogram of 120 patients of the study group and 120 patients of the group of comparison,excluding patients with rheumatic heart disease,cor pulmonale,hypertensive heart disease,myocarditis,myocardiosis,diabetes and thyroid disease,et al.Results There were statistical difference between the percent of E/A〈1 and LAVI between two groups.With the increasing of LAVI,the diastolic function deteriorated.There were 45 atrial fibrillation patients,23 congestive heart failure patients,and 11 stroke patients,6 death patients among study group within the following 3 years.But there was no cardiovascular events among comparison group within the following 3 years.Conclusion Evaluation the changing of left volume with echocardiogram is very important to predict cardiovascular events and to estimate the function of left ventricular diastole.
