近视眼不同屈光状态下调节反应量的观察 被引量:10

Observation on accommodative responses of myopia with different refractive states

摘要 目的分析青年人群中近视眼调节功能与正视眼的差异。方法根据等效球镜度数的不同,将55例(110眼)受试者分为正视组(24眼)、低度近视组(25眼)、中度近视组(36眼)和高度近视组(25眼)。使用GRAND-SEIKO WAM-5500开放视野式自动验光仪测量3D调节刺激下诱发的调节反应量,比较正视眼和低、中、高度近视眼在未矫正和矫正屈光状态下调节反应量的差异。结果正视组诱发的调节反应量为(1.96±0.43)D。在未矫正屈光状态下,低、中、高度近视组诱发的调节反应量分别为(0.34±0.42)D、(0.10±0.27)D、(0.40±0.56)D,正视组与各近视组比较,差异均有显著统计学意义(均为P=0.00),低、中、高度近视组间两两比较,差异均无统计学意义(P=0.20、P=1.00、P=0.05);在矫正至正视状态下,低、中、高度近视组诱发的调节反应量分别为(2.07±0.54)D、(2.06±0.41)D、(1.99±0.53)D,正视组与近视组诱发的调节反应量差异均无统计学意义(F=0.33,P=0.80)。左、右眼未矫正屈光状态下,正视组诱发的调节反应量(1.94±0.40)D与各近视组比较,差异均有显著统计学意义(均为P=0.000),而矫正屈光状态下,差异均无统计学意义(均为P>0.05)。结论青年人群近视眼在屈光不正状态下对调节刺激反应量较低,矫正至正视状态其调节功能与正视眼相近。屈光不正的科学矫正有利于调节的平衡稳定。 Objective To discuss the differences between myopia and emmetropia in accommodative responses among young people.Methods Based on the different SE,55 cases(110 eyes)were divided into emmetropia group(24 eyes),low myopic group(25 eyes),medium myopic group(36 eyes)and high myopic group(25 eyes).Accommodative responses were measured with an objective infrared autorefractor(GRAND-SEIKO,WAM-500)for each eye,the differences among refractive groups with natural and refractive correct status were analyzed in four groups.Results The accommodative responses of the emmetropic group were(1.96±0.43)D,of which the three groups without correcting were(0.34±0.42)D,(0.10±0.27)D,(0.40±0.56)D,respectively,there were significant differences between the emmetropic group and other myopic groups(All P=0.00),but there was no difference among three groups(P=0.20;P=1.00;P=0.05).While the ametropia be corrected,the accommodative responses of the three myopic groups were(2.07±0.54)D,(2.06±0.41)D,(1.99±0.53)D,respectively,there was no difference in all refractive groups(F=0.33,P=0.80).There were significant differences in the accommodative responses between the emmetropic group and other three groups under the natural dual eyes(all P=0.000),but no difference under the corrected dual eyes(all P0.05).Conclusions The accommodative function of myopia ametropia is weaker than that of emmetropia among young people,which be corrected is similar to emmetropia.Ametropia corrected scientifically will contribute to equilibrium of accommodative function and refractive state.
