三阴性乳腺癌超声表现及临床、病理特征

Ultrasonographic features and clinical,histopathological characteristics of triple-negative breast cancer

目的探讨三阴性乳腺癌(TNBC)的超声表现及其临床、病理特征。方法 将318例经病理证实的乳腺癌患者,根据术后免疫组化染色评价雌激素受体(ER)、孕激素受体(PR)和人表皮生长因子受体2(HER2)的表达情况分为TNBC和non-TNBC组,并分析比较其超声表现和临床、病理特征。结果 ①318例乳腺癌患者中,TNBC患者占16.35%(52/318)。②病理结果显示TNBC组腋窝淋巴结转移率高于non-TNBC组(P<0.05)。③超声显示,TNBC组肿块有边缘毛刺征、内部微钙化及彩色多普勒血流成像(CDFI)2～3级的病例数明显低于non-TNBC组(P<0.05)。结论 TNBC腋窝淋巴结转移率较高,但部分患者缺少典型的恶性肿块特征,了解这些特征有助于提高临床及影像学医生对TNBC的诊断水平。

Objective To investigate the ultrasonographic features of triple-negative breast cancer（TNBC） and its clinical and histopathological characteristics.Methods According to the results of immunohistochemical staining technique,318 patients with histopathologically confirmed resectable breast cancer were divided into TNBC group and non-TNBC group.The clinical,histopathological,ultrasonographic characteristics of TNBC patients were analyzed and compared with those of non-TNBC patients.Results ①There were 52 TNBC among 318 patients（52/318,16.35%）.②In TNBC group,the percentage of lymph node metastasis was higher than that of non-TNBC group（P0.05）.③In TNBC group,the percentage of spiculate,microcalcifications and blood stream grade 2-3 were less than those of non-TNBC group（P0.05）.Conclusion TNBC patients have higher lymph node metastasis rate,but most patients lack typical characters of the malignant tumor.Understanding the ultrasonographic characters of TNBC will raise diagnosis level of it.
