迟发性皮肤卟啉症

Porphyria cutanea tarda

报告1例迟发性皮肤卟啉症。患者男,52岁。曝光部位出现红斑及水疱1年余。既往酗酒30年。皮肤科检查:面部、双手背红斑及多发色素沉着斑,左手背见花生米大水疱。皮损组织病理示表皮下水疱和毛虫小体;尿卟啉阳性;诊断为迟发性皮肤卟啉症。予保肝、戒酒及避光后皮损好转。

A 52-year-old male presented with erythema and bullae on light-exposed areas for 1 year. He is an alcohol abuser for 30 years. Physical examination revealed erythema and hyperpigmentation on his face and dorsal hands, a peanut-like bulla presented on his left dorsal hand. Histopathological examination showed a blister under the epidermis and caterpillar bodies in the epidermis. Uroporphyrin was positive. A diagnosis of porphyria cutanea tarda was established. Symptoms were alleviated after giving up drinking and sunlight avoidance.
