孕晚期胎死宫内的原因分析及预防对策

Cause Analysis and Preventive Measures for Fetal Death in Third Trimester

目的探讨孕晚期(孕龄≥36周)胎死宫内的原因及预防对策,以期降低其发生率。方法回顾性分析首都医科大学附属北京妇产医院2003年1月至2010年2月间收治的44例孕龄≥36周的胎死宫内患者的死胎原因。结果孕晚期胎死宫内的主要原因分别是胎儿生长受限、脐带缠绕、妊娠期高血压疾病、脐带过度扭转、羊水过少和胎盘早剥等。结论加强高危孕妇管理和孕期宣教,强调规律产前检查和自数胎动的重要性;积极处理妊娠合并症和并发症,适时终止妊娠。

Objective To investigate the causes and preventive measures for fetal death in third trimester.Methods 44 patients with fetal death（gestational age ≥36 weeks）delivered in Beijing Obstetrics and Gynecology Hospital,Capital Medical University from January 2003 to February 2010 were analyzed retrospectively.Results The main reasons for fetal death in third trimester were fetal growth restriction,umbilical cord entanglement,pregnancy-induced hypertension,umbilical cord too reversed,oligohydramnios and placental abruption.Conclusion To strengthen the management for high-risk pregnant women and the publicity during pregnancy.To emphasize the importance of routine examination and fetal movement counting.To deal with pregnancy complications actively and end pregnancy timely.
