超过精阜的前列腺增生613例电切手术体会

Transurethral resection of the prostate for benign prostatic hyperplasia which was beyond the distal end of seminal colliculus

目的:探讨较大前列腺增生的电切手术治疗方法。方法:对613例超过精阜的前列腺增生患者使用切割前标记,切割中仔细辨认尿道外括约肌的手术方法进行电切手术治疗,分别观察两组术后排尿情况,IPSS评分以及并发症的发生情况。结果:治疗组613例患者术后早期拔除尿管后尿流通畅率为100%,IPSS评分平均下降16.5分,手术前后对比有极显著性差异(P<0.01);无1例发生尿失禁;尿道狭窄发生率为2.28%。结论:术中做好标记;仔细辨认尿道外括约肌;精细电切较大前列腺,可取得较好的手术效果,并可以减少并发症的发生。

Objective： To improve the level of transurethral resection of the prostate（TURP） for benign prostatic hyperplasia（BPH） which was larger than normal.Methods： With the means that label the border of resection,identify the external urethral sphincter carefully,to perform TURP for patients with BPH which was beyond the distal end of seminal colliculus.Had followup after treatment with the situation of urination,IPSS and complication.Results： Within the treatment group（n=613）,patency rate of urination post-operation was 100%;IPSS averagely decreased 16.5 between pre-operation and post-operation,the disparity of statistics about them was significant（P0.01）;none of them had anischuria;incidence rate of angusty in urethra was 2.28%.Compared with control group,none of these items had significant disparity of statistics.Conclusion： This method is valuable to spread because of perfect effect and rare disparity of statistics.
