异丙酚对先天性心脏病小儿体外循环手术脑保护作用的研究

摘要 为观察异丙酚对先天性心脏病(以下称先心病)小儿体外循环(CPB)手术颈内静脉球血8-异前列腺素-F_(2α)(8-iso-PGF_(2α))及S-100β蛋白水平的影响,选择择期先心病小儿30例,年龄4~10岁,随机分为异丙酚组(P组)和对照组(C组),每组15例。P组麻醉诱导用舒芬太尼1μg/kg、异丙酚2.5mg/kg,C组用咪达唑仑0.2mg/kg,两组均用哌库溴铵0.1mg/kg,气管插管后P组静脉泵注1%异丙酚6 mg·kg^(-1).h^(-1),C组静脉泵注0.05%咪达唑仑0.2mg·kg^(-1)·h^(-1),直至手术结束。分别于CPB前(T_0)、CPB开始后30min(T_1)、停CPB(T_2)、停CPB后30min(T_3)、手术结束(T_4)、停CPB后24h(T_5)采颈内静脉球部血检测血浆8-iso-PGF_(2α)及S-100β蛋白水平。结果表明,与C组比较,P组在T_1~T_5时点血浆(8-iso-PGF_(2α))及S-100β蛋白水平降低(P<0.05)。与各自T_0时点比较,S-100β蛋白在T_2时点达峰值(P<0.05),8-iso-PGF_(2α)在T_3时点达峰值(P<0.05)。结论:异丙酚可降低先心病手术小儿CPB期间血浆8-iso-PGF_(2α)及S-100β蛋白水平,具有脑保护作用,其机制与异丙酚抗脂质过氧化作用有关。
