疏肝清障方治疗痰气郁结型精神分裂症的临床研究

Clinical treatment of schizophrenia due to stagnation of phlegm and qi with Shugan-qingzhang formula

目的观察疏肝清障方治疗痰气郁结型精神分裂症的临床疗效。方法将85例精神分裂症患者采用随机数字表法分为两组，治疗纽43例采用白拟疏盯清障方治疗，对照组42例口服利培酮治疗。两组患者均采用阳性与阴性症状量表（PANSS）、中医症状分级量表进行临床疗效评定。治疗后采用副反应量表（TESS）评定药物不良反应。结果两组治疗后PANSS分值均降低，差异均有统计学意义（F治疗组=4．86，t’治疗组=17．88，v’治疗组=59，P治疗组〈0．01；F对照组=4．27，t’对照组=18．76，v‘对照组=59，P对照组〈0．01）；组间PANSS分值下降比较，第2、4周对照组明显优于治疗组（F第2周=1．02，t第2周=2．50，P第2周〈0．05；F第4周=1．04，t第4周=3．55，P第4周〈0．01），第6周组间差异无统计学意义（F=0．92，t=0．91，P〉0．05）；PANSS评分的西医临床疗效比较，差异无统计学意义（x2=0．05，P〉0．05）；中医症状分级量表评分的中医临床疗效比较，治疗组优于对照组，差异有统计学意义（x2=36．26，P〈0．01）；不良反应比较，差异有统计学意义（x2=8．17，P〈0．01）。结论疏肝清障方可在一定时期控制痰气郁结型精神分裂症的症状。

Objective To explore the effects of treating schizophrenia due to stagnation of phlegm and qi with Shugan-qingzhang formula. Methods 85 patients with schizophrenia were randomly recruited into a control group and a treatment group. The treatment group was treated with Shugan-qingzhang formula, and the control group was treated with oral risperidone. Clinical effects of both groups were evaluated by positive and negative syndrome scale （PANSS） and symptom rating scale. Adverse drug reaction was evaluated by treatment emergent symptom scale （TESS）. Results Score of PANSS was significantly changed after the treatment in both groups （F treatment group = 4.86, t＇ treatment group = 17.88, V＇ treatment group = 59, P treatment group = 0.01; , F control group 4.27, t＇ control group= 18.76, V＇ control group= 59, P oontrol group〈0.01）; scores of PAN SS in the treatment group were significantly better than the control group at the second and forth weak （F 2 weeks= 1.02, t 2 weeks=2.50, P a weeks〈 0.05; F 4 weeks= 1.04, t 4 weeks=3.55, P 4 weeks〈0.01）, while no significant difference at the sixth week between the two groups （F=0.92, t=0.91, P〉0.05）; There was no statistical difference of PANSS score between the two groups in terms of western medicine curative effects （x2=0.05, P〉0.05）, but significant difference in terms of TCM curative effects （x2=36.26, P〈0.01） ; There was also statistic difference of adverse drug reaction between the two groups （x2 = 8.17, P〈 0.01）. Conclusion Shugan-qingzhang formula can control symptoms of schizophrenia due to stagnation of phlegm and qi in a certain period.
