亚低温治疗外伤性急性弥漫性脑肿胀38例分析

Analysis of mild hypothermia on 38 cases of post-traumatic acute diffuse brain swelling patients

目的探讨亚低温治疗外伤性急性弥漫性脑肿胀（PADBS）的方法和疗效。方法自2007-09~2009-12对38例PADBS进行亚低温治疗,回顾性分析临床资料。结果 38例急性弥漫性脑肿胀患者,保守治疗23例,中转手术14例,放弃治疗1例。伤后3个月GOS评判：良好21例（55%）,中残10例（26%）,重残3例（8%）,植物生存1例（3%,放弃治疗）,死亡3例（8%）。结论亚低温治疗急性弥漫性脑肿胀简单有效、微侵袭,改善脑代谢状态,减轻急性脑组织损伤的反应程度,利于神经功能恢复和改善预后。

Objective To explore the effect of mild hypothermia on the patients with post-traumatic acute diffuse brain swelling.Methods From September 2007 to December 2009,38 patients with post-traumatic acute diffuse brain swelling were treated with mild hypothermia.The clinical data were retrospectively analyzed.Results Some 23 cases were treated with conservative methods,14 cases were treated by surgery,1 case give up treatment.Three months after injury,21（55%） cases were graded to be good,10（26%） cases with mild disability,3（8%） cases with severe disability,1（3%） case was vegetative and 3 cases（8%） died.Conclusion It seems that mild hypothermia is a good choice to cure post-traumatic acute diffuse brain swelling.It is simple,less invasive,and can reduce brain swelling and play an important role in the protection of neurological function.It is beneficial to neurological recovery.
