脑性瘫痪合并发作性事件动态脑电图分析

Active electroencephalogram monitoring of paroxysmal events for children with cerebral palsy

目的脑性瘫痪儿童临床多出现发作性事件,本研究拟探讨脑性瘫痪儿童发作性事件的临床性质。方法对42例脑瘫合并发作性事件的儿童为观察组,同时选取51例普通儿童伴有发作性事件为对照组。对两组儿童进行24 h动态脑电图监测,探讨发作性事件的临床意义。结果对两组儿童的发作性事件进行比较发现,观察组发作性事件中42.86%有痫样放电,而普通对照组中23.52%有痫样放电。两组比较,观察组发作性事件的癫痫发生率显著高于对照组(χ2=3.937,P=0.047)。进一步对两组的发作性事件类型分析发现,全身性抽搐发作时观察组和对照组痫样放电的发生率差异没有统计学意义(χ2=0.504,P=0.478);而非全身性抽搐发作时,观察组癫痫的比例显著高于对照组,差异具有统计学意义(χ2=7.314,P=0.007)。结论与正常儿童比较,脑性瘫痪的发作性事件中癫痫的比例较高。因此,临床对脑性瘫痪儿童的发作性事件要充分重视,避免漏诊和误诊的发生。

Objective To explore the significance of active EEG（AEEG） monitoring in the diagnosis and differential diagnosis in cerebral palsies children with paroxysmal events.Methods 42 cerebral palsy children with paroxysmal events were enrolled as study group and 51 common children were selected as control group.All children in two groups were measured by AEEG.Results The rate of epileptic discharges in the observation group（42.86%） is higher than that of in the common control group（23.52%）（χ2=3.937,P=0.047）.To take further analysis,it showed that there was no significant difference between the observation group and the common control group when epileptic discharging（χ2=0.504,P=0.478）.Whereas,the epilepsy rate of observation group was significantly higher than that of the common group（χ2=7.314,P=0.007）.Conclusion AEEG monitoring has important values for the diagnosis and differential diagnosis for cerebral palsy children with paroxysmal events.
