恢复期精神分裂症患者阳性强化行为疗法的实施

Implement of reinforced behavioral therapy in schizophrenia patients within recovery phase

目的探讨阳性强化行为疗法对恢复期住院精神分裂症患者临床康复的效果。方法将68例恢复期住院精神分裂症患者采用抛币法随机分为研究组(36例)和对照组(32例),两组患者均给予常规抗精神病药物治疗,对照组采用传统的病房护理管理方法,给予简单的娱乐活动(即非指导性的娱乐活动);研究组在此基础上给予为期8周的阳性强化行为疗法。采用护士用住院患者观察量表(NOSIE)评价两组效果。结果研究组社会能力、社会兴趣、个人整洁因子分和总积极因素分显著高于对照组,而激惹、精神病表现、迟滞因子分以及总消极因素得分显著低于对照组(均P<0.05)。结论阳性强化行为疗法能够矫治精神分裂症患者的行为障碍,对促进患者社会功能的康复,降低精神残疾具有积极作用。

Objective To explore the effect of reinforced behavioral therapy on the convalescence of schizophrenia patients.Methods Totally,68 hospitalized schizophrenia patients within convalescence stage were randomly divided into a study group（36 cases） and a control group（32 cases） by coin-tossing method.The two groups were given conventional antipsychotic drugs.Besides that,the control group were allowed to do simple recreational activities（namely non-directive entertainment activities）,while those of the study group were given reinforced behavioral therapy for 8 weeks.Nurses′ Observation Scale for Inpatient Evaluation（NOSIE） was used to compare the effect between the two groups.Results The scores of social ability,social interest,personal hygiene and total score of positive factors in the study group were significantly higher than those in the control group（P0.05 for all）;the scores of irritability,phychotic manefestation,retardation and total score of negative factors in the study group were significantly lower than those in the control group（P0.05 for all）.Conclusion Reinforced behavioral therapy can correct behavioral disorders of schizophrenia patients.It helps social function recovery of patients and reduces mental disabilities.
