冠心病患者外周血中CD19^+细胞表达水平的意义 被引量:5

摘要 目的观察冠心病患者外周血中CD19+的表达水平,并探讨CD19+与炎症反应和脂质过氧化反应的关系。方法按美国心脏病学会/美国心脏学会冠心病处理指南的诊断标准并经冠状动脉造影检查确诊的冠心病患者60例,分为稳定型心绞痛(SA)组20例,急性冠状动脉综合征(ACS)组40例;并选择冠状动脉造影正常的20例非冠心病患者作为对照组。应用流式细胞术检测所有研究对象外周血中淋巴细胞表面CD19+的表达水平,比较各组CD19+的表达情况。同时检测C反应蛋白(CRP)和低密度脂蛋白胆固醇(LDL-C)浓度。并对CD19+细胞的表达率与CRP滴度和LDL-C进行Pearson直线相关分析。结果 ACS组患者B淋巴细胞CD19+表达率明显高于对照组(10.16±3.63)%vs(4.25±0.96)%(P<0.01),亦明显高于SA组(10.16±3.63)%vs(5.39±1.36)%(P<0.01);SA组CD19+细胞表达率和对照组患者CD19+细胞表达率相近(5.39±1.36)%vs(4.25±0.96)%(P>0.05)。所研究对象外周血中CD19+细胞的表达水平与CRP滴度(r=0.661,P<0.01)和LDL-C(r=0.351,P<0.01)呈正相关。结论随着冠心病病情加重,冠心病患者外周血中B淋巴细胞CD19+的表达水平升高,CD19+的表达水平可反映冠心病危险分层,CD19+与炎症反应和脂质过氧化反应有关系。 Objective To investigate the expression level of CD19^+ in peripheral blood of patients with coronary heart disease(CHD) as well as its role in inflammation and lipid peroxidation.Methods Sixty patients were diagnosed as CHD acocording to the results of coronary angiography and ACC/AHA diagnostic criteria.The patients were divided into two groups:stable angina(SA) group(n=20),acute coronary syndrome(ACS)group(n=40).Another 20 patients with normal coronary angiography served as control group.Expression level of CD19^+ on lymphocyte was detected by means of flow cytometry.Serum C reactive protein(CRP) and low density lipoprotein cholesterol(LDL-C) were also measured.Then correlation analysis was made on the expression level of CD19^+,CRP and LDL-C.Results Expression level of CD19^+ on lymphocyte in peripheral blood was significantly higher in ACS group than in control group(10.16±3.63)% vs(4.25±0.96)%(P〈0.01),and higher than in SA group(10.16±3.63)% vs(5.39±1.36)%(P〈0.01).The expression level of CD19^+ on lymphocyte was similar between SA group and control group(5.39±1.36)% vs(4.25±0.96%)%(P〉0.05).The expression level of CD19^+ on lymphocyte in patients with CHD was positively correlated with CRP(r=0.661,P〈0.01) and LDL-C(r=0.351,P〈0.01).Conclusion The expression level of CD19^+ is significantly increased in patients with ACS and it is positively correlated with serum CRP and LDL-C.
