急性左心衰患者QRS波时限及QTc间期变化的临床意义 被引量:7

摘要 目的探讨急性左心衰患者QRS波时限及QTc间期变化的临床意义。方法将急性左心衰患者158例按预后情况分为预后良好组、预后不良组和死亡组。在入院当时、入院后24、72 h安静状态下观察QRS波时限及QTc间期的变化,采用荧光免疫法检测血浆脑钠肽(BNP)浓度,并分析各指标的相关性。结果入院后预后良好组QRS波时限、QTc间期呈进行性缩短,预后不良组QTc间期呈进行性缩短,死亡组QTc间期呈进行性延长(P<0.05)。在入院后72 h死亡组和预后不良组QRS波时限、QTc间期明显长于预后良好组(P<0.05)。三组患者入院后BNP浓度呈进行性下降(P<0.05)。死亡组和预后不良组患者在三个时间点BNP浓度均高于预后不良组(P<0.05)。QRS波时限和QTc间期分别与BNP和NYHA分级呈正相关(P均<0.05)。结论 QRS波时限及QTc间期在急性左心衰患者病程中的动态变化能反映患者的预后,QRS波时限及QTc间期与心功能相关,可以作为疗效评价的指标。
