股骨近端锁定加压钢板治疗股骨粗隆间骨折的临床分析

Clinical analysis of locking compression plates in the treatment of femoral intertrochanteric fractures.

目的评估股骨近端锁定加压钢板治疗股骨粗隆间骨折的临床疗效。方法手术治疗32例不同类型股骨粗隆间骨折患者,采用股骨近端锁定加压钢板进行内固定。结果 32例均获随访,时间6~14个月,骨折均愈合,平均愈合时间为4.5个月。无畸形愈合、肢体短缩、髋内翻发生,无钢板断裂、松动、断钉及螺钉切割股骨头现象。按Harris髋关节功能评分：优10例,较好14例,良6例,差2例,优良率为93.8%。结论股骨近端锁定加压钢板治疗股骨粗隆间骨折具有术中最大限度保护骨折块血运、手术时间短、骨折愈合快、术后并发症发生率较低、功能恢复满意的特点,尤其适用于老年骨质疏松性股骨粗隆间骨折及不稳定性股骨粗隆间骨折,临床上具有良好的应用价值。

Objective To evaluate the outcomes of locking compression plates in the treatment of femoral intertrochanteric fractures.Methods Thirty-two patients different types of intertrochanteric fracture receiving surgical procedures were treated with locking compression plate for proximal femur fixation.Results Thirty-two patients were followed up for 6-14 months.All fractures healed without malunion,limb shortening,coxa vara,plate fracture and loose,short nails and screws cutting off the femoral head.The average healing time was 4.5 months.According to Harris hip function score criteria： excellent in 10 cases,good in 14 cases,fair in 6 cases and poor in 2 cases,with the effective rate being 93.8%.Conclusion Proximal femur locking compression plate in treatment of intertrochanteric fracture can provide the maximum protection of intraoperative fracture block blood supply,shorten operative time,promote the rapid healing of fracture,reduce the incidence of postoperative complications,leading to functional recovery,especially for osteoporotic intertrochanteric fractures and unstable intertrochanteric fractures in clinical practice.
