Modified incision for the treatment of intra-articular calcaneal fractures.

摘要 目的评价改良切口治疗跟骨关节内骨折的临床疗效。方法采用回顾性研究,我科自2005年1月~2008年12月行跟骨关节内骨折手术75侧,其中41侧使用传统跟骨外侧L形切口,34侧使用改良切口。用SPSS17.0对两者的手术时间、切口长度、切口并发症发生率、B hler角、Gissane角、AOFAS评分做统计学处理。结果两者在手术时间、切口长度、切口并发症发生率P〈0.05,有统计学意义,在B hler角、Gissane角、AOFAS评分P〉0.05,无统计学意义。结论改良切口不但在恢复跟骨解剖结构(B hler角及Gissane角)及关节功能(AOFAS评分)上可以取得和传统切口相同的临床效果,而且可以减少手术时间、切口长度、切口并发症发生率。 Objective To evaluate the clinical effects of modified incision and traditional incision in the treatment of intra-articular calcanea fractures.Methods Seventy-five cases of intra-articular calcaneal fracture were treated with open reduction and interal fixtion by a plate from January 2005 to December 2008.Thirty-four cases were treated with modified incision and 41 cases with traditional incision.Six items of the two groups were evaluated by SPSS17.0: operating time,incision length,incision complication rate,Bhler's angle,Gissane's angle and AOFAS score.Results There was significant difference in operating time,incision length and incision complication rate between the two groups(P0.05),but there was no significant difference in Bhler's angle,Gissane's angle and AOFAS score(P0.05).Conclusion The modified incision not only can achieve the same clinical effects to the traditional incision in reconstructing the calcaneal anatomic structure and recovering excellent weight-bearing function but also can reduce operating time,incision length and incision complication rate.
