塞来昔布对髋膝关节置换手术后出凝血功能的影响

Effect of celecoxib on coagulation function after replacement of total hip and knee.

目的观察塞来昔布对髋膝关节置换手术患者术后出凝血功能的影响。方法于2007年5月至2007年6月选择北京大学人民医院骨关节科收治的行髋膝关节置换手术患者60例,男20例,女40例;年龄23~79岁,平均63岁。按随机数字表法分为塞来昔布组和对照组,每组30例,塞来昔布组麻醉诱导前8h,口服塞来昔布400mg,术后当天可进食后口服200mg,术后第1~5天,200mg/次,2次/天;对照组不服用塞来昔布。观察两组术前术后血小板数量、凝血酶原时间、凝血酶原活动度等情况。结果纳入患者60例,均进入结果分析。塞来昔布组血小板数量、凝血酶原时间、凝血酶原活动度的术前术后变化与对照组相比无显著性差异（P〉0.05）。结论塞来昔布对髋膝关节置换手术患者术后出凝血机能无显著性影响。

Objective To observe the effect of celecoxib on coagulation function after replacement of total hip and knee.Methods Sixty patients subject to replacements of total hip or knee were selected from the Department of Orthopedics,People＇s Hospital Affiliated to Peking University between May 2007 to June 2007.All enrolled patients,20 males and 40 females,aged 23 to 79 years old（average 63）,were randomly divided into 2 groups of 30 each.The study group received a single 400 mg dose of celecoxib,8 h before surgery,and 200 mg of celecoxib every 12 h for 5 days,along with postoperative pain management.The control group received only postoperative pain management.Preoperative and postoperative platelet count,prothrombin time and prothrombin activity were analyzed.Results total of 60 enrolled patients were involved in the analysis of results.There was no significant difference in the variation of preoperative and postoperative platelet count,prothrombin time and prothrombin activity between two groups（P0.05）.Conclusion There was no significant effect of celecoxib on coagulation function after replacement of total hip and knee.
