射频治疗失败后前列腺切除术38例回顾

Prostatectomy after failure of radiofrequency hyperthermia(Report of 38 cases)

目的：探讨前列腺增生症（ＢＰＨ）射频治疗失败的原因及对手术的影响。方法：回顾性分析３８例ＢＰＨ患者射频治疗失败后前列腺切除术的情况及术后前列腺的病理特征。结果：前列腺中叶突入膀胱３０例（７８．９％），前列腺与包膜不同程度粘连１８例（４７．３％），术后并发症１２例（３１．６％），前列腺组织呈慢性炎症２１例（５５．３％）。结论：ＢＰＨ患者射频失败与前列腺大小及突入膀胱程度密切相关，射频后手术难度较大，应注意防治并发症。

Purpose: To investigate the cause of failure of radiofrequency hyperthermia (RF) of benign prostatic hyperplasia (BPH) and its effects on prostatectomy. Methods: The course of prostatectomy and the pathological characteristics of prostate were analysed retrospectively in 38 cases of BPH after failure of RF. Results: In 30 cases (78.9%) the middle prostatic lobe protrudes into the bladder cavity,in 18 cases (47.3%),adhesion of the prostate to the capsule occurs to a varied extent.Postoperative complications appeared in 12 cases (31.6%),prostatic tissues with chronic inflammation present in 21 cases (55.3%). Conclusions: The failure of RF was closely associated with the volume of the prostate and the degree of the prostate protrusion into the bladder cavity.Prostatectomy after RF becomes more difficult and care should be taken to prevent complications.
