膝关节炎患者膝关节滑液中白细胞介素-1β白细胞介素-6含量变化的意义

The significance of the variance of interleukin-1β and interleukin-6 content of knee joint synovial fluid in patients with osteoarthritis

目的检测骨关节炎(OA)膝关节液内白细胞介素(IL)-1β、IL-6的浓度,探讨二者与膝关节炎关节镜下分级的关系,寻找反映骨关节炎严重程度的生化指标。方法选择膝关节镜及人工膝关节置换的患者40例,其中11例单纯半月板损伤为对照组,OA患者29例为实验组,根据其软骨损伤程度分为3组。抽取其膝关节液,采用酶联免疫吸附试验法检测IL-1β、IL-6的浓度。结果对照组与实验组IL-1β浓度差异有统计学意义(P<0.01),实验组不同分级间差异也有统计学意义(P<0.01)。而IL-6检测结果在对照组与实验组之间差异无统计学意义(P=0.09)。两两比较后发现轻度组与中度组中IL-6含量明显高于对照组及重度组(P<0.01),但对照组与重度组之间、轻度组与中度组间差异无统计学意义(P>0.01)。OA患者关节液中IL-1β的含量与关节镜下OA的严重程度成正相关(r=0.887,P<0.01),IL-6含量与OA的严重程度无直线相关关系。结论 OA患者关节液IL-1β含量明显高于健康者,可作为区分OA严重程度的标志物。OA患者关节液中IL-6含量早期明显升高,晚期呈下降趋势,不能作为区分OA严重程度的标志物。

Objective To detect the interleukin（IL）-1β and IL-6 concentrations of knee fluid in patients with osteoarthritis（OA）,and to explore the relationship between the grading of knee arthroscopy and IL-1β,IL-6 and to seek the valuable biochemical indicators.Methods Forty patients suffered arthroscopic knee replacement were selected,of which 11 cases with simple meniscus injury were taken as control,and the others（n=29） with OA were as the experi-mental objectives.According to their degree of cartilage injury,patients in experimental group were divided into three groups（mild group,moderate group and sever group）.Collected their knee joint fluid,and concentrations of IL-1β and IL-6 were detected with ELISA method.Results IL-1β concentration was significantly different between the control group and the experimental group（P〈0.01）,and significant differences existed between different grades of the experimental group（P〈0.01）.There was no significant difference in the IL-6 test results between the control group and experimental groups（P=0.09）,and IL-6 levels in the mild group and moderate group were significantly higher than that in the control and the severe group（P〈0.01）,but the differences between control group and severe group,mild group and moderate group were not statistically significant.IL-1β levels in synovial fluid of OA patients were positively correlated with the severity of OA arthroscopy（r=0.887,P〈0.01）,there was no linear relationship between IL-6 levels and OA severity.Conclusion Synovial fluid IL-1β in OA patients was significantly higher than that in healthy persons,and can be taken as a marker reflecting the severity of OA.Synovial fluid IL-6 levels in OA patients significantly increase in early stage and decline in later stage,and can＇t be taken as a marker of distinguishing the severity of OA.
