超声引导穿刺注射5-Fu术联合中药治疗异位妊娠的研究

Study of 5-Fu Injection Therapy Combined with Herbal Treatment for Ectopic Pregnancy Under the Guide of Ultrasound

目的观察超声引导穿刺注射5-Fu术联合中药治疗异位妊娠的临床疗效。方法将60例异位妊娠患者随机分为A组和B组各30例,A组采用腔内超声引导细针穿刺异位妊娠囊注射5-Fu术联合中药,B组采用腹腔镜下5-Fu局部注射术联合中药。观察两种方法治疗异位妊娠的综合疗效、治疗后的临床症状、手术时间、出血量、术后下床活动时间、术后发热情况、胃肠功能恢复情况、术后镇痛情况、术后住院日及术后妊娠情况等的变化。结果在手术时间、术后发热例数和术后腹痛消失时间上,两组比较差异有统计学意义(P<0.05),A组优于B组。在肠道功能恢复时间上两组比较差异有统计学意义(P<0.01),A组明显优于B组。A组输卵管通畅率16/26(61.54%),术后2例宫内妊娠,妊娠率2/26(7.69%);B组输卵管通畅率15/25(60.00%),术后1例宫内妊娠,妊娠率1/25(4.00%)。两组输卵管通畅度及妊娠率比较,差异无统计学意义(P>0.05)。治疗过程中未发现明显毒性反应。结论超声引导穿刺注射5-Fu术联合中药治疗异位妊娠具有较好的临床疗效,是临床治疗异位妊娠的有效方法。

Objective To observe the clinical effect of 5-Fu injection therapy combined with herbal treatment for ectopic pregnancy under the guide of ultrasound. Methods Sixty ectopic pregnancy patients were randomly divided into group A and group B（each n=30）.5-Fu injection therapy combined with herbal treatment was executed in group A and 5-Fu local injection therapy was individually executed in group B under laparoscopy.The differences in the clinical effect,clinical symptoms,operation time,blood loss,evacuation active time,postoperative fever, restoration of gastrointestinal function,postoperative analgesia,postoperative hospital stay,and postoperative pregnancy were compared between the two groups. Results There were statistically significant differences in operation time,the sum total of postoperative fever and postoperative abdominal pain disappear between the two groups（P0.05）,and group A was superior to group B.There were statistically significant differences in the time for restoration of gastrointestinal function between the two groups（P0.01）,and group A was significantly superior to group B.The tubal patent rate in group A was 61.54%（16/26）,and intrauterine pregnancy was obtained in 2 patients,with the pregnancy rate of 7.69%（2/26）. The tubal patent rate in group B was 60.00%（15/25）,and intrauterine pregnancy was obtained in 1 patient,with the pregnancy rate of 4.00%（1/25）.No statistically significant difference was found in tubal patent rate and pregnancy rate between the two groups（P0.05）.No toxicity response was found in the process of the operation. Conclusions Better clinical curative effect was obtained by 5-Fu injection therapy combined with herbal treatment for ectopic pregnancy under the guide of ultrasound.It is an effective way in clinical therapy for ectopic pregnancy.
