挫伤性视网膜脱离疗效相关因素分析

The analysis of correlative factors of retinal detachment caused by contusion

目的分析影响挫伤性视网膜脱离疗效的有关因素。方法回顾了96例视网膜破裂的部位、类型、外伤到网脱的间隔时间，手术复位率及视力。结果在视网膜破裂的病例中，17例巨大裂孔及锯齿缘离断，其中12例位于颞下象限，53眼不规则裂孔。伤后1周内诊断出破裂性视网膜脱离8例，伤后1周～6周内对例。39．3％手术成功，35例视力0．1以上。结论视网膜脱离在外伤后6周内诊断出其视力预后较好。

Objective The correlative factors of contusive retinal detachmentwere evaluated. Methods The location and the types of retinal breaks,the time interval from contusion to recognition of retinal detachment,reattachment rate and, visual aruity results of these 96 cases are re-viewed. Results Of the retinal breaks recognized giant tear and dialpsisat the ora serrata were oberved in 17 eyes,of which 12 were situated atthe lower temporal quadrant. Fifty-three eyes had irregular breaks Eightpatients had retinal breaks or retinal detachment diagnosed within oneweek of contusion abd 39 within six weeks. Surgical treatment successful-ly restored or maintained retinal apposition in 93 .3% of the eyes.Thirtyfive eyes recovered visual aculty of 0. 1 or better,of which 30 eyes withinsix weeks. Conclusion Visual prognosis was good when retinal break ordetachment were diagnosed withen six weeks of contusion. However,thosepatients who escaped initial retinal examination and were lost to follow -uphad a less favorable visual outcome.
