带血供胫骨内侧中、上段骨膜瓣移位术的应用解剖

Applied anatomy of transposition for periosteal flap of middst and upside of tibial medial with vessels

目的为带血供胫骨内侧中、上段骨膜瓣移位术提供解剖学基础。方法在４０侧灌注红色乳胶的成尸标本上，对胫骨内侧中、上段骨膜血管的来源、走行、分支、分布及其吻合进行观察，并在标本上摹拟术式设计。结果胫骨内侧中、上段骨膜由隐动脉、膝下内动脉，胫后动脉肌间隙支和胫前返动脉分布。其外径分别为（１．７±０．４）ｍｍ、（１．２±０．３）ｍｍ、（２．１±０．７）ｍｍ和（２．３±０．６）ｍｍ。结论以上述血管之一为蒂设计骨膜瓣移位修复膝周骨缺损的术式，具有可行性。

Objective\ To provide an anatomical basis for transposition for periosteal flap of midst and upside of tibial medial with vessels.Methods\ The origin, course, branches, distribution and anastomosis of periosteal vessels of midst and upside of tibial medial were observed on 40 sides of adult cadavers with red latex in artery. The operation of simulation was devised on the specimens.Results\ The saphenous artery, medial inferior genicular artery, septum intermuscular branch of posterior tibial artery and anterior tibial recurrent artery distributed periosteal of midst and upside of tibial medial with the external diameter of 1.7 mm ± 0.4 mm, 1.2 mm ±0.3 mm, 2.1 mm ± 0.7 mm and 2.3 mm ± 0.6 mm,respectively.Conclusion\ The periosteal flap with one of aforesaid arteries can be transposition to repair the bone defect around the knee joint.
