摘要
目的:观察各种恶性肿瘤及相应良性病变中 D N A 含量及倍体的变化,分析其对诊断、分级和预后判断的价值。方法:利用 D I P A S200 生物图像处理分析系统,对常见恶性肿瘤及其良性病变共500 例作 I C M D N A 分析研究。结果:(1) 宫颈鳞癌、卵巢常见恶性肿瘤( 浆液性及粘液性囊腺癌) 、滤泡性淋巴瘤的 D N A 含量和超二倍体率均显著高于相应起源组织或良性肿瘤;(2) 结直肠高分化腺癌与良性病变之间、各种类型甲状腺癌与正常甲状腺滤泡上皮和良性肿瘤之间的 D N A 含量和倍体无显著差异;(3) 乳腺浸润性导管癌、宫颈鳞癌、结直肠腺癌的分化程度越低, D N A 含量均越高,超二倍体率也越高。结论: I C M D N A 检测能在定量水平分析肿瘤,可有助于良恶性肿瘤的鉴别,特别是对一些交界性肿瘤和临床疑难病例的诊断可提供有价值的参考指标, D N A 含量和 D N A 倍体特征与某些恶性肿瘤的分化状态有关。
Objective:To analyze DNA content and the ratio of hyperdiploid and their value in diagnosis and prognosis of malignant tumors and benign lesions. Methods:DNA image cytometry (ICM DNA) was applied to analyze 500 cases of malignant tumors and corresponding benign lesions.Results:(1)DNA contents and the ratio of hyperdiploid were higher in cervical squamous carcinoma, serous and mucinous ovarian cystadenocarcinoma and follicular lymphoma than those of their corresponding original tissues or benign lesions.(2)No significant difference in DNA contents and the ratio of hyperdiploid was found between well differentiated colon or rectum adenocarcinoma and benign lesions and between all types of thyroid carcinoma and normal thyroid follicular epithelium or benign tumors.(3) In invasive breast ductal carcinoma, cervical squamous carcinoma and colon or rectum carcinoma,the lower differentiation, the higher DNA contents and the ratio of hyperdiploid were found. Conclusions:DNA quantitation can be analyzed using ICM DNA.ICM DNA is helpful in differentiating malignancy and benignity.It can especially provide valuable reference indices for differentiation in some borderline neoplasms and difficult cases.DNA contents and the characteristics of the DNA ploidy are associated with the degree of differentiation in some malignant neoplasms.
