子宫动静脉套扎术在腹腔镜下子宫肌瘤剔除术中的应用

Application of loop ligature method of uterine arteries and vein in operation of hysteromyomas resecting by laparoscope

目的探讨子宫动静脉套扎术在腹腔镜下子宫肌瘤剔除术中的应用价值,以解决腹腔镜下行子宫肌壁间肌瘤剔除术中出血较多、手术时间长的问题。方法 A组(87例):腹腔镜下先行子宫体下段动静脉暂时套扎再行子宫肌瘤剔除手术;B组(85例):腹腔镜下直接行子宫肌瘤剔除手术,对收集到的数据采用SPSS13.0软件进行统计分析。结果 两组病例在手术时间、术中出血量有显著性差异(P<0.05),术后住院日,术后应用抗生素,术后并发为无显著差异,结论A组与B组比较,由于很好的解决了腹腔镜下子宫肌瘤剔除术中出血的问题,减少了止血的时间,而且操作简单,能够明显减少出血量、缩短手术时间。

Objective To investigate the effects of loop ligature method of uterine arteries and vein in operation of hysteromyomas resecting by laparoscope.Methods 172 patients were divided to A group（87 cases） and B group（85 cases）.In A group,blocking uterine arteries and vein with loop ligature was applied before resecting hysteromyomas by laparoscope;In B group,directly resecting hysteromyomas by laparoscope.Results There was statistical significant difference in the average operative time and the averaged operative blood loss（P0.05）,no significant difference in the mean length of hospitalization,the therapeutic antibiotics schedule and the incidence of operative complications（P0.05） between the groups A and B.Conclusions Laparoscopic myomectomy with loop ligature method is a safe,ideal method with many advantages,such as less bleeding and less operative time.
