两种麻醉方法对肥胖患者手术期胰岛素抵抗的对比研究

摘要 目的:比较丙泊酚复合芬太尼静脉输注与异氟醚吸入麻醉对肥胖患者手术期胰岛素抵抗的影响。方法:54例肥胖患者随机分为A、B两组,每组各27例,A组术中采用丙泊酚复合芬太尼手控输注;B组采用异氟醚吸入麻醉为主的麻醉方式。于术前30min及术毕60min各抽取静脉血8ml测定血糖和血清胰岛素,计算胰岛素抵抗指数与胰岛素分泌指数并进行比较。结果:两组患者术毕60min胰岛素抵抗指数与胰岛素分泌指数均较术前30min增大,P值均<0.05;B组患者术毕60min胰岛素抵抗指数与胰岛素分泌指数均较A组增大,P值均<0.05。结论:丙泊酚复合芬太尼静脉输注可抑制患者术中应激反应,对降低肥胖患者胰岛素抵抗程度的作用优于异氟醚吸入麻醉。
