摘要
目的 探讨应用应变-血压指数(SBPI)评价健康成年人颈总动脉弹性的临床价值及可行性.方法 随机选取健康成年人208例,均应用应变和应变率成像、射频动脉僵硬度分析技术对颈总动脉进行检查.通过应变和应变率成像获得颈总动脉的最大圆周应变,同时测量被检者肱动脉血压,通过射频动脉僵硬度分析技术获得颈总动脉的顺应性系数(CC)、僵硬度系数(β)以及局部血压.计算颈总动脉的SBPI,并将SBPI分别与CC、β进行相关性分析;后将被检查者按照不同年龄分组,进行各组间的对比分析.结果 颈总动脉的SBPI与CC呈显著正相关,而与β呈显著负相关(均P<0.01).20~29、30~39和40~49岁组的SBPI、β、CC的差异均有统计学意义(均P<0.01);但50~59岁的β和CC参数与40~49岁组之间的差异无统计学意义(P>0.05),而其SBPI与40~49岁组之间的差异有统计学意义(P<0.01).20~24、25~29、30~34、35~39、40~44、45~49、50~54和55~59岁组中,β和CC两项参数在相邻两组间差异均无统计学意义(均P>0.05),而SBPI在相邻两组间差异则有统计学意义(P<0.01).结论 SBPI可作为评价颈总动脉弹性的新指标,并且较β和CC更精确、敏感.
Objective In this study, we aimed to investigate the clinical value and the feasibility of strain-blood pressure index(SBPI )in assessing the elasticity of common carotid artery in healthy adults. Methods 208 healthy adults(20 to 59 years old) examined in our ultrasound department from April 2009 to September 2009 were enrolled in this study. Maximum of circumferential strain (CSmax) of common carotid artery of each case was acquired through strain and strain rate imaging (Xstrain), and blood pressure of brachial artery was measured at the same time. Compliance coefficient (CC),Stiffness index (β) and the local blood pressure of carotid artery were acquired through Quality arterial stiffness (QAS) software. Strain-blood pressure index (SBPI) of common carotid artery was calculated. Results There was significant positive correlation between SBPI and CC, and there was significant negative correlation between SBPI and β ( both P〈0.01 ).SBPI, β and CC had significant difference among 20-29 years age group,30-39 years age group and 40-49 years age group (all P〈0.01 ). β and CC had no significant difference between 40-49 years age group and 50-59 years age group (P 〉0.05 ), while SBPI had significant difference between 40-49 years age group and 50-59 years age group (P〈0.01). Among 20-24 years age group,25-29 years age group,30-34 years age group, 35-39 years age group,40 to 44 years age group,45-49 years age group,50-54 years age group and 55-59 years age group, 13 and CC had no significant difference between adjacent age groups (P 〉0.05 ), while SBPI had significant difference between adjacent age groups(P〈0.01 ).Conclusion SBPI might be a new index in assessing the elasticity of common carotid arteries accurately and sensitively.
