腮腺多形性腺瘤手术治疗和并发症的防治

Surgical Treatment for Pleomorphic Adenoma of the Parotid Gland and Prevention Measures for the Complications after Parotidectomy

目的探讨腮腺多形性腺瘤手术治疗和并发症的防治。方法对97例腮腺多形性腺瘤做保留面神经和腮腺浅叶切除术及保留腮腺导管+腮腺区域性切除术。结果暂时性面瘫15例,复发1例,涎瘘2例,皮下积液10例,Frey′s综合征8例,耳垂麻木10例。结论腮腺多形性腺瘤切除+腮腺区域性切除术是有效的术式,有利于保留残余的腺体功能及面部的美观,防止并发症的发生。

Objective To study the surgical treatment for pleomorphic adenoma of the parotid gland and prevention measures for the complications after parotidectomy.Methods In 97 cases with pleomorphic adenoma of the parotid gland,superficial lobes were ectomized with remaining cranial nerve or regional parotidectomy were made with remaining conduit.Results After operations,temporary Bell palsy occurred in 15 cases,tumor recurrences occurred in 1 case,salivary fistula occurred in 2 cases,salivary dropsy occurred in 10 cases,Frey′s syndromes occurred in 8 cases,numbness of ear lobe occurred in 10 cases.Conclusion Excision of pleomorphic adenoma and surperficial lobe of the parotid gland were effective treatment to reserve functions of the remaining gland and keep beautiful outlook and prevent the complications after parotidectomy.
